Step 2
Please provide all the information requested below. If you are not sure which version (how leads exit ballast) your ballast is, click on the Version Sheet [PDF].   Submitting a completed form aids in timely processing of your warranty claim.  If you need assistance, please contact Robertson's Warranty Department at 800-323-5633, dial 5 for the Warranty Department.

If you have verified the date code information and are certain that the product is defective and has not been abused or operated improperly, please submit this warranty claim form.(PDF)  . You can email the completed form to warranty@robertsonww.com or print the form and fax it to the Warranty Department at 877-388-2420.
